Housers are increasingly reaching out to health care partners to help their residents stay better connected to care for behavioral or physical health conditions that can cause housing instability. Health care providers are also finding these partnerships help meet their goals of reducing use of crisis and emergency services, keeping people out of nursing homes and other institutional settings, and better overall health outcomes for residents. Come and hear about these partnerships and the primary care and wellness services health care partners can deliver for affordable housing residents. Presenters will share how to approach health care partners, what it takes to collaborate with the health care system, and the outcomes they’ve achieved.
